The risk assessment form worksheet sizes and prices a firm's whole assessment programme rather than one document: the activities you have to assess split into low, medium and high effort, the minutes each of those takes to write properly, the months between scheduled reviews and what share of the writing time a review costs, the assessments a change forces out of cycle in a year, the people every change has to reach and the minutes briefing one of them takes, and what the hour costs you. It returns the hours and the money to write the set from scratch, the hours a year on scheduled reviews and on out-of-cycle changes, the briefing time nobody counts, how many assessments fall due in an average month, and what carrying one assessment costs you a year.

Which figures do I need before using the risk assessment form worksheet?
Your own activity list split three ways by how much work each assessment is, the minutes each takes you to write, your review cycle in months, roughly how many assessments a change forced out of cycle last year, the number of people a change has to reach, and what an hour of your time costs the firm. Nothing comes from outside your own organisation.
Why does it count briefing time separately?
Because it is usually the largest number on the page and the one nobody budgets. Revising an assessment takes an hour; telling twenty-two people what changed, in a way you can evidence, takes considerably longer, and an assessment nobody has read has changed nothing on the floor. Putting the two side by side is the point of the worksheet.
Does the worksheet write my risk assessment?
No, and it does not tell you whether your controls are adequate. It sizes and prices the programme as a piece of work. What goes in a particular assessment is a judgement about your workplace, made by someone competent who has seen it, and the HSE's own guidance is explicit that copying an example and putting your company name on it does not meet the duty.
Where the constants in this tool come from

What the tool assumes a written assessment records, taken from the regulator's own template: who might be harmed and how, what you are already doing to control the risks, what further action you need to take, who needs to carry out that action, and when it is needed by. The same page offers six worked examples and warns in terms not to copy one and put your company name on it, which is why this site prices the writing rather than supplying the words.
Management of Health and Safety at Work Regulations 1999, regulation 3 (legislation.gov.uk). The duty the programme exists to meet in Great Britain: every employer shall make a suitable and sufficient assessment of the risks, and shall review it if it is no longer valid or there has been a significant change. `No longer valid` is why the worksheet asks how many assessments a change forced out of cycle, rather than assuming an annual round.
