A safety file is easy to start and almost impossible to keep, and keeping it is the part that gets asked about

Each assessment, sheet and incident as its own record, with who wrote it and when

A folder of documents is not a file. A record with an author and a date can be questioned, defended and improved; a Word file called RA_final_v3.docx on a shared drive can only be argued about. The name on the record is what turns a document into evidence that somebody competent did the work.

The next review on every record, before it lapses rather than after

Reviews slip because they are diary events attached to nothing. When the review date lives on the record, the month's work is a list of what is owed rather than a memory of what was done, and an assessment nobody has looked at since the process changed shows up as overdue instead of as silence.
